Dental + vision care
You can afford good health
Sound too good to be true? Anthem offers these benefits to our members through Indiana’s Healthy Indiana Plan (HIP) Plus program. You only pay a small amount, as low as $1 per month, based on your income. If you don’t want to make a monthly payment, there is another option called HIP Basic available to some members.
HIP Plus vs. HIP Basic
HIP Plus offers the best value — no copays, plus dental, vision, chiropractic care and extra pharmacy benefits! HIP Basic has no monthly payment, but you’ll pay copays for services, like doctor visits and prescriptions, and you don’t get dental, vision or chiropractic care.
Do you qualify?
If you answer yes to these questions, you may qualify:
- Do you live in Indiana?
- Are you between the ages of 19 and 64?
- Is your monthly income less than the amount below?
You can also use Indiana’s HIP Eligibility and Contribution Calculator to see if you qualify and to find out how much your monthly payment would be for HIP Plus.

What happens next?
After you are approved by FSSA, you’ll get a welcome packet and an invoice to make your first monthly payment. Be sure to make your payment on time to enroll in HIP Plus. We’ll let you know when you can start using your benefits.
What happens if you don’t make your first payment?
- If you qualify for HIP Basic, you’ll be enrolled in that program. You’ll have to pay copays for your health care services and you won’t be able to join HIP Plus for a year.
- If your income is too high to qualify for HIP Basic, you will not be enrolled in any health coverage and will have to reapply.
